Onondaga Community Health Survey

Dear Families and Staff Members - 

Every three years the Onondaga County Health Department conducts a Community Health Assessment and Improvement Plan in partnership with local hospitals. To inform this process, we’ve developed a Community Health Survey to hear directly from Onondaga County residents.  We would like your help to broaden our reach!  Please distribute this survey to colleagues and families so we can get a variety of perspectives from all those we serve.   
 
The survey is open to all Onondaga County residents aged 18 years and older. 

  • The survey takes about 10-15 minutes.        
  • It is voluntary and responses are confidential.
  • Participants can enter for a chance to win a $200 VISA gift card.
  • The survey is open through July 15, 2025.
